When to Pick This
Mid lane Alistar is at his best when the enemy mid laner has no dash and no hard escape, since Headbutt only needs to connect once to end their game entirely. An immobile mage or a channel-reliant caster on the other side of the lane cannot get away from a knockback that throws them into a river bush or straight at a jungler.
He is also a strong pick when your own jungler plays an early skirmish or invade heavy style rather than a farm to six style, since Pulverize and Headbutt both work better as tools that turn a 1v1 into a 2v1 than as tools for winning a lane by himself. Pair him with a jungler who wants early fights and this pick starts looking a lot less strange.
How It Works
Ratios Built For A Mage Item Path
Pulverize scales at 80% AP and Headbutt scales at 100% AP, both far higher than most of the ratios support Alistar ever gets to use with a Shurelya's and a Redemption in his inventory. Land both in the same combo behind a Luden's Echo proc and a stacked Rabadon's Deathcap and the damage reads a lot closer to a real mid lane burst mage than a peel bot.
Cooldowns Built For Roaming
Both Pulverize and Headbutt drop to a ten second cooldown at max rank, and neither one costs enough mana to keep him tethered to the wave. That leaves him free to shove and walk down a side lane on a timer most mages cannot match, arriving with a full CC kit instead of just a hopeful ultimate.
A Gank Setup No Other Mid Laner Has
Flash into Pulverize catches a target with zero warning, and Headbutt right behind it throws them straight into whatever your jungler is standing in. Landed correctly, that combo is close to a guaranteed kill with almost any jungler on the roster, not just the ones built around burst.
Potential Pitfalls
No Real Escape Before Level Six
Headbutt is a dash toward an enemy, not away from one, and Pulverize has almost no range. Until Unbreakable Will comes online at level six, an assassin or a burst mage that all-ins him first has no real punish to worry about, so the early laning phase has to be played around not getting caught rather than winning trades outright.
Damage Ceiling Is Lower Than A Real Mage
Even with strong ratios, Alistar's base damage on all three abilities is still tuned for a champion that was never supposed to solo carry a lane. A fed hypercarry mid mage will still out-damage him in a straight up late game teamfight, so this pick wins games by controlling fights rather than by winning a damage race.

Mid lane Alistar is not trying to out-mage a real mage, and trying to force that comparison misses why the pick works. He wins by roaming on a timer nothing else in the role can match and by turning Headbutt into a gank tool that erases a target from the map before their team even reacts. Play him next to a jungler who wants to fight early and this weird pick starts looking like a legitimate lane bully with a very unusual toolkit.
